Taxi booking form template

A taxi booking form needs only what the dispatcher must know: pickup, destination, date and time, passenger name and phone, number of passengers and luggage, vehicle type, and any notes such as flight number or wheelchair. Every extra field loses bookings, and a form with live prices and instant confirmation converts better than one that waits for a callback.

Key facts

Booking details most councils require operators to record
Pick-up point (286 of 289 councils), destination (285), pick-up time (280), passenger name (279)[1]

Which fields does a taxi booking form need?

The ones your licence requires you to record plus what the driver needs: pickup, destination, date and time, passenger name and contact, passengers and luggage, vehicle type and notes. In England and Wales almost every council requires operators to record the pick-up point, destination, pick-up time and passenger name.[1]

The HTML form template

Paste it into your website and point the action at your booking handler or email service. It collects a request; a dispatcher still has to confirm it.

Why do plain booking forms lose customers?

Because the customer leaves without a price or a confirmation and books whoever answers first. A booking widget that shows the price for each vehicle, takes payment or account details and confirms instantly turns the same visit into a booking that lands straight in dispatch.

The TaxiDex web booker embeds in your website, prices the trip from your own fare rules, takes card or wallet payment, looks up flights, and drops the booking straight into dispatch.

Frequently asked questions

Is there a taxi booking form plugin for WordPress?

Generic form plugins can send booking requests by email; a dispatch system's embeddable booker goes further by pricing, confirming and dispatching the booking. The HTML above works in any WordPress HTML block.

Should the booking form take payment?

For pre-booked work, taking a card or account at booking cuts no-shows and disputes; pair it with a clear cancellation policy.
